Digital Delivery
Most program materials, intake forms, client communications, reports, and educational resources may be delivered digitally through email, client portal, secure upload links, or other approved software systems.
Physical Kits
If an HTMA hair sample kit or other physical material is included, it may be shipped to the mailing address provided during enrollment. Clients are responsible for providing accurate shipping information and promptly notifying Zen Quantum Wellness of any address changes.
Shipping Timelines
Shipping and laboratory processing times vary. Zen Quantum Wellness cannot guarantee delivery or processing dates because timelines may depend on carriers, laboratories, client response time, holidays, weather, inventory, and regulatory requirements.
Lost or Delayed Shipments
If a kit is lost, delayed, damaged, or not received, please contact Zen Quantum Wellness. Replacement availability may depend on the circumstances, carrier status, laboratory policy, and any costs already incurred.
State and Jurisdiction Restrictions
Some tests or specimen submissions may not be available in every state or jurisdiction. New York and other jurisdictions may have additional laboratory requirements. If testing cannot be fulfilled because of legal or laboratory restrictions, Zen Quantum Wellness may provide alternate instructions, modify the program flow, or address the issue under the applicable refund policy.
Client Responsibilities
Clients are responsible for completing forms, collecting samples correctly, mailing samples promptly when required, uploading requested documents, and responding to reasonable administrative requests needed to deliver the program.
